The surgery is now accepting new patient registrations.


Please note, that currently due to the Coronavirus outbreak we will be registering patients without the need for them to attend the surgery however we ask that you provide electronic copies of ID (if you are able to) when you register.

Please note that BOTH forms must be completed for you to be registered.

Patients wishing to register with the surgery are asked to complete a GMS1 Form and the New Patient Questionnaire Form. This is to make sure that we receive enough information about you to start providing appropriate care as soon as the process is complete.


Please note that you will need to provide the necessary ID in order for the registration to be complete.  Please provide photo ID (current passport or photo Driving License) and proof of your current address (Utility Bill / Tenancy Agreement etc.). You can email your proof of ID to

If you are unable to fill out the forms online you can come to the surgery and collect a paper copy, which you can take away and fill out.  When you bring the completed forms back to the surgery please bring 2 forms of ID as mentioned above.
