Complaints & Feedback

The practice operates an in-house complaints procedure in order to deal with any complaints that are made by patients or their representatives.  We take all feedback (both positive and negative) and complaints seriously and use them as tools with which we can monitor, evaluate and improve the services we offer our patients.  The Business Manager, Bret Stevenson,, is the complaints manager at Park Crescent Health Centre

From June 2021 we are trialing a new Complaints Form template. This has been developed to try and support patients to get across their issues, but also what action they would like seen taken by the practice as a result. Please provide as much information on the complaint form as you can. If you wish to just send in a written complaint then this is fine as well, but the questions on the complaint form may help you. There are hard copy complaint forms  available from reception.
