Patient Participation Group

 

 

smiling_patientWe are always looking for new members of our Patient Particpation Group.  We would like to work with our patients to gain feedback from you about your experiences, improve communication, make the most of practice resources, review our services, help us to make positive changes and promote good health.

If you interested in joining the group or finding more out about it, please download  and complete the patient group flier and put it into the Patient Group box on the reception counter next time you are in or post it to us.  You can email the forms back to the practice using the following email address: mailto: bhccg.parkcrescenthc@nhs.net

 

The minutes of our last patient group meeting along with a number of other reports and newsletters are available to download from the above “News & Reports” tab.
